Passport Requirements

Complete Passport application. (You will need Adobe Acrobat to view the application. If you do not have Acrobat on your computer, click here for free download.)

Bring the following items when applying for a new passport:

Original Birth Certificate with raised seal
Two 2" x 2" photos
Marriage License (only if present name is different than one on driver's license)

Signature on application must be witnessed by a clerk in the Circuit Clerks' Office
Cost: $97.00 (Check or money order in the amount of $67 payable to the US Departmetn of State plus cash, check,or money order in the amount of $30 payable to the Scott County Circuit Clerk.)
Applications can be expedited with additional costs of $60.00 plus two overnight delivery fees.
Passports are valid for ten years.

When applying for child under 14 years of age, both parents must sign application and present photo ID, or one parent can submit notarized authorization for other parent to apply for passport for the child.
Child must be present.
Children's passports are valid for only five years.

To Renew Passport:

New 2" x 2" photos
Previous passport
Must have been at least 16 years old when most recent U.S. passport was issued.
Most recent passport must have been issued less than 15 years ago.
